Thank you for your interest in Ebbs & Flows Counseling.  By completing this form, we are better able to provide you with appropriate client-centered care.  Once you have completed this form, a member of our staff will reach out to you within 2 business days to discuss options for starting therapy, being placed on a waitlist or providing additional recommendations.
